General Physician Information

Under the "General" tab of the New Referring Physician task, enter the basic demographic information on the physician, including:

  • Full Name: Enter the referring physician's full name (e.g., John L. Davidson, Robert Peterson, etc.). Enter the suffix, if available.

  • Date of Birth: This information is typically not required by insurance payers.

  • NPI: Enter the referring physician's NPI number. Note: Many insurance payers do require this information on both paper and electronic claims. To avoid claim rejections, it is important that you record this number on the referring physician's record.

  • SSN: Enter the referring physician's Social Security number, if available.

  • Specialty: Click the Specialty button and select the referring physician's main specialty from the list of Taxonomy Codes maintained by the system. Some insurance payers do require this information.

  • Degree: Enter the referring physician's degree (e.g., MD, DO, PhD, MET, etc.).

  • Type: Leave this field blank. This field relates to providers within the practice only, and not to outside referring physicians.

  • Address: Enter the referring physician's office address. This could be important information to obtain should this physician need to be contacted by mail in the future.

  • Email Address: Enter the referring physician's email address (e.g., JBrown@medicalpractice.com), if available.

  • User: Leave this field blank. This field is used for providers within the practice who use the Kareo Practitioner software. This field is not used for outside referring physicians.

  • Home: Enter the referring physician's home phone number, including area code, if available.

  • Work: Enter the referring physician's office phone number, including area code and extension. This could be an important phone number to have on record should this physician need to be contacted by phone in the future.

  • Mobile: Enter the referring physician's mobile phone number, including area code, if available.

  • Pager: Enter the referring physician's pager number, including area code, if available.

  • Fax: Enter the referring physician's fax number, including area code, if available. This is another important number to have on record should you or another member of the billing company or practice need to fax documents to this physician in the future.

  • Notes: Enter any ad-hoc notes or other data that you wish to store with the referring physician record.
